Lactulose is a non-absorbable synthetic disaccharide that reaches the colon unchanged, where it is fermented by intestinal bacteria into organic acids. These acids increase the osmotic pressure within the colon, drawing water into the bowel and softening stools. In hepatic encephalopathy, the acidic environment converts ammonia (NH₃) into ammonium (NH₄⁺), reducing its absorption into the bloodstream and lowering blood ammonia levels.
